Hi,
I think the best way to be approached is casually. A lot of girls are on their guard against creeps and ignore those who are overly direct. But most will be pleased to be asked out for coffee or lunch, because it's the kind of date that's not quite a date, ie. there are no expectations.
I personally think the best thing to wear is, again, casual. Jeans and a t-shirt that fit you will let the girl know that you take care of yourself, whereas oversized clothes can be intimidating or sloppy. And make sure you comb your hair--that's definatley something a girl notices.
